Both are tools for eliminating ground loops, which cause hum or buzz in an audio system when two pieces of equipment have ground reference at slightly different voltages. An isolation transformer physically breaks the electrical connection between input and output while still passing the audio signal, eliminating the loop entirely; this is the most reliable solution. A ground lifter is a switch that disconnects the audio cable's ground at one end; cheaper but less reliable, and can introduce safety issues if misused. Use isolation transformers in permanent installations where any hum is unacceptable.
